Advance care planning is a recommended way of expressing autonomy by long-term care patients, and it also applies to the growing population suffering from dementia. Advance care planning and setting its goals mean decision-making autonomy of patients and constitutes the basis for providing holistic and personalized care. It is a process involving a dialogue between the patient and his relatives on the one hand, and the healthcare staff and other people involved in care on the other. The interviews held should result in determining the therapeutic and caring procedure to be taken in case of a deterioration of the patient's health and their loss of ability of decision making. Those arrangements should be based on the values, beliefs and goals previously defined by the patient. This form of care planning allows both patients and their loved ones to be prepared for changes caused by progressive dementia. The thesis is aimed at presenting some selected issues related to the application of the concept of advance care planning in the case of patients suffering from dementia disorders.
